Additional survey address

For those who may have received bounces when trying to send me mail for the
survey (haven't heard of any, but one never knows), mjb has been kind enough
to set up a forwarding address on hoosier.utah.edu that will get your info
to me.  The address is:

        sol-survey@autox.team.net
