Doug,
If this is a manifold as opposed to the header, the area
to which you speak has a "D" shapped port, and over they years,
someone may have felt that the #1 and #4 port which share
this issue, needed a little opening/rounding up.
Perhaps they got over zealous, and broke through?
Its pretty tough to weld iron, consider replacement.

Subject: Exhaust manifold
A hole erupted in my exhaust manifold.
It is next to the machined surface for the stud on the left.
The area is quite thin of metal and the hole got bigger as I cleaned it up.
The hole is now the size of a pencil eraser.
I try a high heat putty compound from the inside... but that did not work.
Ideas?
