Throw-out Bearing Replacement Recommendations?

I'm doing a general overhaul on a '74 TR6 engine & trans (I was gonna wait
for winter, but the clutch fork pin and TO bearing failed).

Question 1: What's the best option for a release (throw-out) bearing
replacement?

Question 2: Should I replace the sleeve/carrier, too? It has a couple of
small dings, but looks serviceable.
